Your Initial Visit
To prepare for your first visit with us, you’ll need to provide us with some basic demographic information about yourself and any insurance plan under which you might be covered. For your convenience, we offer an online patient portal for you to register as a new patient. By completing your information online, you’ll avoid having to arrive early for your appointment to complete paperwork. If you do not feel comfortable using our online portal, please feel free to download the patient registration forms to complete and bring in to our office.
When you come in for your first visit, you’ll need to bring the following items:
- Photo ID
- Insurance Card
- Prescription / Order for Therapy (From Your Doctor)
- List of Medications You Are Currently Taking
- Any X-Rays, MRIs, etc.
Lastly, be sure to dress comfortable in loose-fitting clothing as you will be performing exercises. Your initial visit with us will last somewhere between one hour to 90 minutes.
Your therapist will determine the number of visits needed for treatment of your condition. Be sure to attend your scheduled subsequent visits. Your active participation in your therapy sessions is pertinent to your recovery. We do understand that things come up. Should you need to cancel or reschedule an appointment, please contact our office within 24 hours of your scheduled appointment.
